Have a LifeSpan expert nurse coach you and/or your parents through an illness episode. This assistance package includes an initial interview with your parent, family and health providers by telephone or in person, a review of available records and a summary submitted directly to you in a way you can understand. The summary outlines your parent’s present condition, documents the current providers and treatment plan, and highlights your parents’ and family goals for future care. The assessment also makes recommendations to address the patient and family centered health goals, and estimates what services may be required to meet those goals.

After the initial evaluation, additional support and services are available by the hour to help you control expenses. Coordination of care activities are largely completed via telephone and correspondence, and personal contact is maintained with the client where beneficial to your mother or father’s condition (e.g., doctor appointments where necessary). Payment for fees are requested at the time that the service is delivered, and payments can be made by Visa or MasterCard for your convenience.
