The Way We Use Information
We use the information that you voluntarily provide about yourself (and, where applicable, your family members) only in connection with the production and support of your LifeSpan services. The personal information that you provide typically includes your name, address, email address, phone number, credit/debit card information and personal health information. With the following exceptions, we do not share this information with outside parties:  to comply with the law, to enforce or apply our Terms and Conditions, or to protect the rights, property or safety of LifeSpan Care Management, LLC or our clients. We use email addresses supplied by clients: to provide confirmation of transactions and client-related information; to answer email that we receive from clients and other site visitors; and to alert clients and other visitors of special LifeSpan promotions. Such email addresses are not shared with outside parties, except as described above. We periodically employ third parties to perform various functions on our behalf, for example, website hosting, data analysis, processing credit card payments and providing client service. These third parties will have access to your personal information solely as may be necessary for them to perform such functions. At our discretion, we may use non-identifying and aggregate information (i.e., non-personal information) to better design the Website and to share with our business advisors for strategic planning purposes. You can register with our Website if you would like to opt-in to receive our newsletter, as well as updates on our new products, services and promotions.  How to Enter and Update Information
LifeSpan Care Management, LLC will record client personal information provided upon initial order of the LifeSpan GuardianKey™. Thereafter, LifeSpan clients may add/update personal information, including medical information, by mail, fax, or email.

Use of Information Upon Termination.
You may terminate your client status at any time by submitting written notice, either by email or mail, to: LifeSpan Care Management, LLC, The Waterfront Technology Center at Camden, 200 Federal Street, 2nd Floor, Camden, NJ 08103. Upon receipt of notice of termination, LifeSpan Care Management, LLC is released from any and all responsibility and/or liability due to the accessibility and use, or lack thereof, of your LifeSpan GuardianKey™.  information. LifeSpan Care Management, LLC will delete the electronic back-up copy of your LifeSpan within 30 days of the date of termination. LifeSpan Care Management, LLC is under no obligation to keep your information and records subsequent to termination.

 Children's Privacy
We feel that protecting children’s privacy is especially important. For that reason, we never collect information from those whom we actually know are under age 18. Parents or guardians of children under the age of 18 may register them for our service and should manage the personal health care information for such a child directly.

Notification of Material Changes
If LifeSpan Care Management, LLC makes any material changes in our privacy practices, we will post a prominent notice on the Website informing clients of the change. In some cases, we will also email clients who have opted to receive communications from us, notifying them of the changes in our privacy practices.

Client Feedback
Clients should submit feedback relating to the LifeSpan CareKey or the LifeSpan Website. All submitted feedback becomes the property of LifeSpan Care Management, LLC. Feedback is considered non-personal information, and as such, we may use feedback at our discretion for marketing purposes or to contact you for further comments on LifeSpan or the Website. If you provide feedback and we are able to identify you, we will contact you for approval before using your communication for marketing purposes.
