What is "Care Management"?
The main service of LifeSpan Care Management is ‘Care’ Management, as differentiated from Case Management. The definition of Case Management, according to the authorities at the Individual Case Management Society and the Case Management Society of America, “Case Management is a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual’s health needs through communication and available resources to promote quality, cost-effective outcomes.” Case Management has evolved in accordance with the requirements of the payer of health benefits – the insurance companies. Case Management has come more concerned with finances than health care.
Unlike our competition, LifeSpan’s Care Managers work for the patient, not for the system. They are all Registered Nurses, so, unlike many of the laymen representing themselves as Case Managers, our nurse Care Managers relate as a peer to physicians and other providers. Their professional experience in the medical world makes them highly effective in ascertaining expected recovery time frames, what practical issues the patient and their family will have to encounter, and what realistic health and functional outcomes the patient will look forward to. Because they have expertise in the type of injury or illness they are managing, they are able to anticipate potential problems with the patient’s condition and deal with them proactively.
In reality, Case Managers are employed by insurance companies, hospitals or nursing home-type facilities, and so answer to those organizations, not the patient or their family, and are not given any incentive to have the best interests of the patient at heart. The Case Management Society of America (CMSA) states that these practitioners are not ‘Case Managers’ as they just function in a Utilization Review Capacity and do not manage the patient across the entire continuum of care. Their main concern is to manage costs, not overall care. Many of these Case Managers are not even nurses, but social workers.

GuardianKey is easy.
We do it for you. When you order your LifeSpan
GuardianKey, we will collect a number of important pieces of information from you. We combine the information that you provide with information gathered from your actual medical records to create your LifeSpan
GuardianKey.
What You Get With Your GuardianKey
GuardianKeys are available in a variety of styles, including Swivels with neck lanyards, necklaces, wristbands, and keychain models. Although additional services are available with different levels of LifeSpan’s Concierge Service, GuardianKey, SafetyKey and GroupCare, all LifeSpan Subscribers receive: - The USB key of your choice (makes and models vary, and may change without notice.)
- A laminated LifeSpan Subscriber ID Wallet Card with your password
- (2) Window stickers for your auto and/or home to alert EMT’s and emergency personnel that you are a LifeSpan subscriber so that your medical records can be made available quickly in the event of emergency.
- A Client Update Record Form to be filled out by you or your doctor, and mailed back to us with your GuardianKey to update your USB device (Click here to get a copy)

Maintaining Your Medical Records on Your LifeSpan GuardianKey
The LifeSpan GuardianKey’s ability to update itself is one of its most exciting features. It is important that you keep the information on your LifeSpan GuardianKeyTM current. Certain information on the LifeSpan GuardianKey is provided by you — insurance information, emergency contacts, your living will, etc. You may update this information by mail, fax, or phone.
The rest of the information comes from your doctors. So when you have a medical visit that produces new information for your LifeSpan GuardianKey (i.e. an annual physical, change of medications, lab work, etc.) simply ask that your doctor to mail or fax the notes from the visit to LifeSpan GuardianKey on your behalf.
Once we receive your new information, we review it and post it to be loaded onto your LifeSpan GuardianKey.
If your LifeSpan package entitles you, we will update your LifeSpan GuardianKey for you. When you receive your LifeSpan GuardianKey, you will receive a set of LifeSpan GuardianKey update forms and a LifeSpan GuardianKey address label. Simply complete the update form, and mail it along with your LifeSpan GuardianKey to us. We recommend that you insure your package for the value of your GuardianKey service, and request Delivery Confirmation service if using USPS. We will update your LifeSpan GuardianKey and quickly return it to you. Top
Accessing your medical records in an emergency
99% of all doctors' offices and medical facilities have at least one computer onsite. To access your medical information, doctors simply plug your LifeSpan GuardianKeyTM into the USB port on their computers and view your records using Adobe Acrobat Reader or any other PDF format reader. All GuardianKeys contain a link to download and install this software for free. Internet access is NOT required for your doctors to access your records on the LifeSpan GuardianKeyTM; however, internet access is required to download the free reader software. Top
Your privacy and the security of your information:
If you lose your LifeSpan GuardianKeyTM, notify us immediately either by phone, email, or through the LifeSpan GuardianKeyTM website.
Your information is in your control. We maintain a back-up copy of your LifeSpan GuardianKeyTM in the event that it is damaged or lost - and to enable us to manage your updates for you. However, this information is NOT accessible for viewing online. Your LifeSpan GuardianKeyTM is designed to provide doctors and emergency medical personnel with easy access to your medical information - even if you are unable to speak for yourself. The files on your LifeSpan GuardianKeyTM and SafetyKey TMare password protected and comply with HIPAA regulations. You password will be provided on a laminated subscriber wallet card. If you are unable to provide your password to an authorized health professional in the event of emergency, they can call us Toll-Free at 1-800-808-9844 24 hours a day. Your doctor or health provider must supply us with proper authorization from you to have us share your records with him in the event he is unwilling or unable to use the LifeSpan GuardianKeyTM or SafetyKeyTM .
If there is certain information that you do not want on your LifeSpan GuardianKeyTM , you may state your preferences when you order your LifeSpan GuardianKeyTM — or you may choose to remove information from your LifeSpan GuardianKeyTM at any time.
