LifeSpan Care Management LLC is a consortium of highly skilled (MSN, NPs or specialty certified) nurses who can assist you by handling the problem, or by coaching you through handling the most problematic patient/family issues. We assist with coordination and advocacy services, with our hourly charge paid by the family.

As a trusted advisor to your client and their families, you know that getting the right information at the right time can be critical to the outcome of almost any situation.
